Early Results of Autologous Cultivated Limbal Stem Cell Transplantation in Total Limbal Stem Cell Deficiency

Abstract: PURPOSE: To report the early results of transplantation of autologous limbal stem cells cultivated on amniotic membrane (AM) in patients with total unilateral limbal stem cell deficiency (LSCD). METHODS: Four eyes of 4 patients with total unilateral LSCD confirmed with impression cytology underwent transplantation of autologous limbal stem cell cultivated on AM. At each follow up visit, a complete eye examination with special attention to recurrence or regression of vascularization, corneal opacification, and epithelial defect healing was performed. Digital imaging was performed at each follow up visit. Impression cytology was repeated in all cases after surgery. RESULTS: The patients were followed for 5-13 months. Visual acuity improved in all cases. Decrease in corneal opacification and vascularization was obvious in 3 cases with coverage of the cornea with corneal epithelium. Sectoral conjunctivalization was evident in these 3 cases, however the corneas were ready for transplantation. The procedure failed in one case with total corneal conjunctivalization. CONCLUSION: Transplantation of autologous stem cells cultivated on AM seems to be an effective way for total LSCD. More definite judgment needs longer follow up together with long-term results of corneal transplantation in these patients.
Anterior Chamber Contamination at the Conclusion of Phacoemulsification

Abstract: PURPOSE: To evaluate anterior chamber aspirates at the conclusion of phacoemulsification and intraocular lens implantation (PE+IOL) for bacterial and fungal contamination. METHODS: We prospectively evaluated 80 eyes of 80 patients undergoing routine PE+IOL by performing bacterial and fungal culture on aspirates obtained from the anterior chamber at the end of the surgery. RESULTS: Anterior chamber fluid aspirates were positive for bacteria in 5 eyes (6.33%) with coagulase-negative staphylococcus being the most common organism (three eyes). No instance of positive fungus culture was observed. One of the culture-positive eyes developed postoperative uveitis which resolved during a week of treatment with topical corticosteroids and antibiotics. None of the eyes developed endophthalmitis. CONCLUSION: In the current series, the rate of anterior chamber contamination by bacteria at the end of phacoemulsification was in the lower range reported by previous studies.

Abstract: Kidney transplantation in patients with end stage renal diseaseis preferred to dialysis because transplantation provides a betterquality of life and improved survival. However, the gapbetween the supply and demand for a renal allograft is wideningand the waiting time is increasing. Iranian protocol, a controlledtransplant program supported by the government forliving unrelated donors, was initiated for solving the problemof organ shortage. Although this system might experiencechallenges, clearly it has advantages over other organ procurementsystems primarily that thousands in need do not diewhile waiting for a compatible donor.In the present review I discuss the history of renal transplantationin Iran, "Iranian model" protocol, the situation ofIran’s kidney transplantation from either living or deceaseddonors compared with the Middle East countries, and our experiencesof unrelated renal transplantation.

Abstract: Background: Cytomegalovirus (CMV) infection is a common complication following kidney transplantation.Objective: To assess the incidence and risk factors of CMV infection among renal transplant recipients.Methods: In a retrospective multicenter study, 3065 renal transplant recipients from 17 transplant centersof Iran were studied between April 2008 and January 2011. Kidney transplant patients were routinelymonitored by sequential blood samples drawn for use in the CMV-pp65 antigenemia assay, and forhematological and biochemistry tests.Results: 63% of studied patients were males; the mean±SD age of participants was 38±15 years. Themajority of cases (81%) received a kidney from a living unrelated donor (LURD), 9% from living relateddonor (LRD), and 10% from deceased donors. 671 patients experienced CMV viremia. The incidenceof CMV infection was 21.9% (95% CI: 20.4%–23.4%). The rate was higher in the first 6 months aftertransplantation (p<0.001); in recipients with higher level of cyclosporine (p<0.001); in those with lowerhemoglobin concentration (p=0.02); patients with elevated ALT (p<0.001); those with increased fastingblood sugar (p=0.005); recipients with dyslipidemia (p<0.05); deceased kidney recipients (p=0.006); andpatients with kidney graft impairment (p=0.01). In multivariate regression analysis, time since kidneytransplantation (p<0.001) and renal allograft failure (p<0.001) were the only risk factors associated withCMV infection.Conclusions: CMV infection was a common complication in the first 6 months of kidney transplantation,particularly among patients with kidney graft impairment.

Abstract: Behzad EinollahiNephrology and Urology Research Center, Baqiyatallah University of Medical Sciences, Tehran, IranI recently read with great interest the review article "Epidemiology and treatment approaches in management of invasive fungal infections" by Kriengkauykiat et al1 that was published in your journal. This review drew attention to the steadily growing number of invasive fungal infections (IFIs) that are due to the increasing number of severely immunocompromised patients. Despite advances in antifungal regimens in terms of prophylaxis and treatment, IFIs may lead to high mortality rates in solid organ recipients.
